Hello Cab Students and Parents,
Is your child thinking about playing a sport next year? Is your summer booked with lots of activities? Then, now is the time to have your child get their Sports physical done for next school year and if they are a member of the Wellness Center, there is no cost to you!!! Attached is the application for the Wellness Center, the DIAA physical form, and the instructions.
All students playing a fall sport must have their paperwork turned in by August 1, 2017. Please do not hesitate to contact me with any questions.
